The Fine Print

Members may cancel their subscription at any time.

Membership rates are subject to change with the yearly adjustment of the practice fee schedule - members will be informed of new rate 2 months in advance.

The applicable fee for any restorative service (not in the list of included services) is due on the date of service unless specific financial arrangements are made with the administrative staff prior to service.
